Phyllis Ann Welch Lewis was born May 24, 1956 in Maraine, OH to the union of Eula Issac Welch Russell and the late Lyndon Welch. She departed this walk of life on Sunday, April 21, 2013 at the Pikeville Medical Center at the age of 56 years. She spent her formative years in Ohio and currently, she was residing at Thousandsticks, KY at the time of her passing. Phyllis was a homemaker and was of the Pentecostal Faith. She was a member of the Thousandsticks Volunteer Fire Department. She had been a member there since 1997. Also, Phyllis was a Kentucky Colonel and a former member of the Helping Hands. Phyllis enjoyed collecting, reading, crafts, ceramics and most importantly, helping others. Phyllis is preceded in death by her father: Lyndon Welch. Phyllis Ann Lewis leaves surviving the following relatives: Her loving and devoted husband: Purl Lewis of Thousandsticks, KY; Mother: Eula Russell; 1 son: Dean Lewis (Tracy) of Thousandsticks, KY; 2 sons she loved as her own: Brandon McFarron and Eric Morgan; 2 brothers: Steven Welch (Rita) of Maraine, OH and Jason Lynn Welch (Michell) of Mckee, KY; 1 step sister: Stacy Russell Collett (Greg) of Hyden, KY; Special friends: Brandy Pence, Anna Carol Jones and Bessie Collins; A host of nieces, nephews, other relatives and friends also survive her passing. Funeral Services will be conducted at 11:00 A.M. - Thursday, April 25, 2013 at the Thousandsticks United Methodist Church with Covy Asher, Robert Amundson, Roger Wolfe and John Newell officiating. Burial will follow in the Lewis Cemetery at Thousandsticks, KY. Nephews will serve as pallbearers. The Lewis family will receive friends for visitation after 6:00 P.M. - Wednesday, April 24, 2013 at the Thousandsticks United Methodist Church.
